cannot accept clients whose Blue Cross or Cigna policies are HMOs; for HMOs you must contact your Primary Care Provider for
a referral within the HMO.
If your insurance provider is not listed on this page, payment is due at the
time the services are rendered, and you will be responsible for filing with your insurance company. I will provide you with
an invoice suitable for submission to your insurance company.
